June 2009 Nurses Initial Registration Schedule for Davao and Lucena

Nurses (davao examinees only) shall take their initial registration on:

1. Abadies – Berano / 08-03-09
2. Berdul – David / 08-04-09
3. Dayday – Gatchalian / 08-05-09
4. Gatmaitan – Lozada / 08-06-09
5. Lu – Pagaduan / 08-07-09
6. Pagalan – Sarabosquez / 08-10-09
7. Sarenas – Zarate / 08-11-09

Successful examinees should personally register and bring the following
requirements to facilitate registration:
a. Two (2) pcs. passport size colored pictures, white background, in proper attire (with collar & with sleeves) and with complete nametag (for the Oath Form and Certificate of Registration).
b. One (1) pc. – 1” x 1” picture, white background, in proper attire (with collar & with sleeves) & with complete name tag (for the Registry Leaf).
c. 2009 Community Tax Certificate (Cedula).
d. PhP 1,050.00 payment (PhP 600.00 – Registration fee; PhP 450.00 – Annual Fee).

Nurses (lucena examinees only) shall take their initial registration on:
